Referrals

Multiple Healthcare Support aims to match appropriate and suitable accommodation to meet the specific needs of the young people and adults. All enquiries can be made to our office via telephone discussions and / or emails. We will conduct our own assessment. We welcome social workers, families and prospective young people and adults to visit prior to admission.

All admissions will comply with the supported living best practice procedures and we will encourage all stakeholders to attend transition meetings which are held before admission. However, Multiple Healthcare Support considers emergency admissions in special situations. Great care is taken to assess any emergency referrals to ensure the balance within the existing group of young people and adults is maintained. Multiple Healthcare Support aspires to provide a tailored transition plan for everyone before admission.
